If glasses and/or contact lenses are worn, current glasses and most recent eyeglass prescription/passport.
If you wear contact lenses, please come with glasses and bring the contact lenses with you, the eye test will be done with glasses first, then with contact lenses.
If you need glasses for the first time, please bring an eye exam report. You can find the form for this under the following link: www.lab.de, it must be filled out completely! This also applies if the type of glasses is changed, e.g. from distance glasses to varifocals.

For the initial examination always an ophthalmological examination report . The form for this can be found at the following link: www.lab.de,
it must be filled in completely! This also applies if you need glasses for the first time or if you change the type of glasses, e.g. from distance glasses to varifocals.
